WHAT SHOULD I PREPARE FOR MY INITIAL CONSULTATION?
Prepare yourself to be more healthy and happy as you begin to take more care of yourself. Before our initial consultation you will complete a questionnaire to inform me about your health past and where our journey together will begin.
